If using the email option, you must complete the form, and then send as an attachment. Applications will not send directly from this website. All applications sent by email will receive a confirmation of receipt. If you do not receive a confirmation, we did NOT receive your application. Each Act or Group must submit a separate application. Multiple entries on one application are not permitted.

*ALL BANDS MUST SUBMIT A VIDEO TAPE or DVD of the band members performing a song they wish to perform for consideration. We will NOT be auditioning Bands live. The name of the Band, the names of all members of the band, and the song information MUST be included with this application to be considered and cannot be changed once submitted. Band will only be allowed to perform ONE song. if chosen.
Please include any information, background, training, etc., about your act to be used by the emcee in your introduction. Please be complete.
Please list ALL technical requirements such as microphones, piano, music ? If technical requirements are not listed they may not be available for you.
Please consider my/our application for participation in Wintertainment sponsored by the Pendleton Heights High School Performing Arts Department. I/ we understand the artistic performance considered, will be of a nature to be shared by the entire family. I/we understand that, screening will be at the discretion of the Wintertainment committee during auditions on January 20th, if needed. If selected, I/we will be at a dress rehearsal on the evening of Wednesday, January 27th and the performance on the evening of Thursday, January 28, 2010. I/we also understand this is a donation of time and talent and no payment or stipend will be received. I/We release South Madison School Corporation and its representatives, as well, as parents and students involved with Winterainment, of any liability for injury or theft during the dress rehearsal or performance.
